How do I book an appointment?

You can email your requisition form to oshawaultrasound@gmail.com or have your doctor fax your referral form to (905) 576-0798, as soon as we receive the referral we are going to give you a call to find a suitable date and time for you to come to our clinic. You are always welcome to give us a call at (905) 576-2622.

We prefer to receive the form first so that we know exactly what kind of ultrasound was requested. Please note that during the pandemic we do not accept walk-in patients.

What happens if I need to reschedule/cancel my appointment?

We all know that sometimes emergencies come up quickly, but please try your best to call our office 24 hours in advance.
